This will allow us to develop a custom aromatherapy blend to rebalance your body, mind and spirit.

- This consultation is to assist selecting the right essential oils and products for your condition.
- This is a guide only and any problems arisen from your diagnosis will not be the responsibility of 'Aroma Spa'.
- We advise that you use only relaxing/very safe oils from our chart.
- Full consultation guidance can only be assured when the client is under the treatment of a aromatherapist, where the client can be fully assessed.
- No part of this form or its contents can be used without the prior approval of Aroma Spa.
